My current fee is based on a sliding scale starting at $100.00 for a
fifty-minute individual session, $120.00 for couples and families and
$150.00 for the initial intake session. This is determined by you or
your family's gross monthly income. If your financial situation changes,
please let me know so that I can renegotiate your fee. I accept payment
for counseling at the beginning of each, session by cash or check,
unless otherwise agreed. I am currently in the process of being able
to accept credit cards. Due to the nature of my practice, court reports,
depositions and/or court appearances are not possible. If either becomes
mandatory however, there will be a $200/hour charge.
If you have coverage
through an insurance company or out of network counseling service coverage
or have an Employee Assistance Program
for which I am a preferred provider, you may pay only the co-pay
amount at the time of the first session. However, please contact your
insurance
company prior to the first session and obtain specific information
regarding co-pay amount, deductible, eligibility and benefits and
bring a current insurance card showing the, eligibility/benefits phone
number,

I require 24 hours notice for cancellation or
rescheduled appointments. Without this, I will ask for payment in full
for the
missed session.
If you are late for a session, you will be given the time remaining
in your hour, and will be charged the full rate. I also charge
for any counseling and/or consultation that take place over the phone.
This will be billed at the same rate as in the office.
